Case Manager

Health Services, Inc. is a progressive healthcare organization specializing in innovative quality patient care, integrating physical, mental and social well-being. HSI has served the community for over 50 years by providing patients with a full range of quality comprehensive services. We have 11 clinics located throughout the River Region, 2 mobile clinics, and 4 school-based clinics in partnership with Montgomery Public Schools.
HSI is looking for a Case Manager to join our team in Montgomery, AL. Our Case Managers play an integral part of our team, providing clinical and clerical support to our Providers and patients.
Responsibilities:
- Provide case management services to patients in need, including assessment, planning, and coordination of services
- Conduct regular meetings with patients to assess progress and update service plans
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for patients
- Advocate for patients' needs and rights within the community and social service systems
- Develop and maintain accurate and up-to-date client records
- Provide crisis intervention and support to clients in emergency situations
- Utilize motivational interviewing techniques to engage clients in goal-setting and behavior-change
Skills:
- Familiarity with social work principles and practices
- Ability to provide mentoring and support to individuals facing challenges
- Proficient in crisis intervention techniques
- Understanding of behavioral health issues and treatment options
- Experience in addiction counseling or related field is a plus
- Knowledge of workforce development programs and resources
